Includes needs assessment completion, provision of knowledge and education regarding self advocacy methods within the healthcare and mental health system, alongside other life navigation tools for up to three 60 minute sessions per month.  For the person who says, "I need somebody to help me figure out what is going on and where to start." 

Includes collaborative identification of resources, needs assessment completion, preparation and coaching through healthcare and mental health systems, as well as assistance in setting and achieving realistic goals for up to six 60 minute sessions per month. For the person who is saying, "I understand some of what is happening, but I need help keeping everything organized and moving" 

Includes services from Inform and Navigate tiers for unlimited, pre-scheduled, 60 minute sessions per month at client request.  For the person or family who is saying, "There are too many moving pieces. I need someone helping me stay on top of this."
